What You’ll Need to Get Started
Basic Information
We’ll need some general details about the parent/caregiver and the individual receiving services to start the intake process.
Insurance Card
A clear photo of the front and back of your insurance card helps us check your coverage and benefits quickly.
Referral for ABA Services
A prescription or referral from a pediatrician, neurologist, or doctor is often required by insurance. Don’t have one? We can guide you!
Diagnostic Reports
If available, diagnostic reports confirming a qualifying diagnosis (such as autism or developmental delays). These are usually conducted by a licensed psychologist, neurologist, or developmental pediatrician. Not sure if you have this? Don't worry, we can help you!
